Konnect II offered by POCAAN

Helps and assists HIV+ individuals of color who are newly diagnosed, at-risk or not receiving care. Refers to health and social service resources, education, information and weekly support groups.

Programs that develop plans for the evaluation, treatment and/or care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual. Case management is a collaborative process characterized by communication, advocacy and resource management to promote high quality, cost-effective interventions and outcomes.

Case/Care Management

Programs that provide information regarding practices that reduce the risk of an individual contracting an infection, including AIDS, through sexual activity.

Safer Sex Education

Programs that provide information about drug use (including the drugs most commonly used and their effects, the symptoms of related disorders, screening and diagnostic procedures and methods of treatment) and/or offer any of a variety of services that focus on drug use prevention for people of all ages who are at risk (i.e., who are currently in the light or experimental stage of using drugs; who are members of a drug using sibling, peer or family group; and/or who have behavioral and/or emotional problems at home, in school, in the community or with the criminal justice system). Included may be printed materials or videos that address the subject; psycho-educational and skill building activities; structured groups which focus on family dynamics, problem-solving, self-esteem and similar issues; and presentations in schools and agencies and to family groups regarding the dangers of drug use, the signs of related disorders, the legal consequences of drug use and how to get help.

Drug Use Disorder Education/Prevention

Mutual support groups whose members are people who have a disability, illness or other health condition, their families and friends. The groups meet in-person, by telephone or via the Internet; and provide an opportunity for participants to share information, resources, practical tips for daily living and encouragement about issues related to the disability or health problem.

Health/Disability Related Support Groups

Programs that offer HIV tests which are used to identify individuals who have been infected with the Human Immunodeficiency Virus (HIV) and are at risk for developing AIDS (acquired immune deficiency syndrome) or which are used to measure progression of the disease in people known to be infected. The most common HIV screening test is the enzyme immunoassay (EIA) which most frequently uses peripheral blood drawn from the arm or a finger as a sample, but can also be conducted using serum, oral fluids or urine. Repeatedly reactive EIA tests are confirmed using the Western blot or the immunofluorescence assay (IFA). The most common test that is used to measure disease progression is the PCR (polymeraise chain reaction) or viral load test. Many programs that provide HIV testing also provide pre-testing and post-test counseling which includes information about AIDS/HIV, reducing risks for HIV transmission, emotional support to help the individual deal with the testing process and test results, and information about and referral to other AIDS-related services.

HIV Testing

Mutual support groups whose members are people who have any type of behavioral addiction or a substance use disorder involving alcohol or other drugs. The groups meet in-person, by telephone or via the Internet; provide emotional support, information and resources to help those who participate modify their behavior; and may include faith-based and secular 12-step groups as well as non-12 step groups.

General Addictions/Substance Use Disorder Support Groups

Programs that intercede on behalf of individuals to help them establish eligibility for or obtain needed services when they have been denied benefits or services for which they are eligible, when they need assistance to communicate their needs to a service provider or to otherwise effectively represent themselves, or when they have a complaint about a service. Individual advocacy attempts to reach equitable settlements without resorting to litigation and seeks to meet individual needs without attempting to change social institutions.
