Serves those who are pregnant or have children under 1 yr old that receive medical insurance through the State of Washington. Provides professional in-home visits by a staff of licensed nurses, counselors, and dietitians for low income or at-risk pregnant women who are facing barriers to having a healthy pregnancy. Works with women during pregnancy and, depending on her needs, into her baby's first year of life. Each client is provided with one-on-one counseling and education, in her home or place of residence, to help her have a healthy pregnancy and take needed steps to prepare and provide for her family. Also assists with resources for housing for homeless clients (when funding allows), life skills classes, community events, vital needs (such as diapers, car seats/cribs and food), job training, and many other services and resources to help women succeed. Moms can also continue to attend events and gatherings, even after they are not currently in the program. May also provide information on Safe Sleep and access to cribs to those receiving services.

Children with Special Health Care Needs - CSHCN Connects families of children with special health care needs to providers and services. Provides case management, care coordination and referral to community-based programs. Maternal Child Outreach Team - MCOT Works to reduce high rates of infant mortality in Pierce County. Connects newly pregnant mothers to health insurance including Medicaid, smoking cessation and other support services. Maternity Support Services - MSS Promotes healthy pregnancies and babies through nurse home visiting. Provides preventative health and education services to women who are enrolled in Medicaid. Nurse Family Partnership - NFP Provides nurse home visiting services for first-time pregnant low-income women. Offers special health care need for children with disabilities and the First Step program. Promotes an integrated system of services for infants, children and youth up to age 18 years who have or are at risk for chronic physical, developmental, behavioral, or emotional conditions and require health and related services of a type or amount beyond what is generally needed. The nurse is able to help families navigate through complex medical systems and connect with resources. Provides home or office nursing visits for assessment, referral and health education through the second month after the baby is born.

Offers universal home nursing/home visiting program. Provides preventative health and education services to help a woman to have a healthy baby. Includes helping families learn about and how to use needed medical, social, educational and other resources in the community so the baby and family can thrive. Also offers weekly support with a virtual and in-person Baby Lounge - a place for all families with babies to connect and learn together as parents. Groups are facilitated by a health professional. There are child care stipends available, and refreshments are provided.

First and second-time moms are partnered with a registered nurse early in their pregnancy and receive ongoing nurse home visits through their child’s second birthday. Nurses provide health and developmental screenings to identify potential problems with growth, hearing, social-emotional development, oral health, and other issues. Nurses also offer breastfeeding support, parenting support, and general parenting education.

Specially trained Community Health Nurses visit families in their home throughout the pregnancy and continue to visit until the child is 2 years old. The nurses visit weekly and bi-weekly during the pregnancy, infancy and toddler period. The education curriculum includes information during each phase of the development. This relationship based model builds on the parent’s strengths supporting the parent child relationship, healthy parent-child outcomes and goals for the future helping first time parents succeed.
